The Challenge You're Facing

Your Crews Are Managing Behavioural Crisis Without the Training or Legal Framework to Do It Safely

Crews Face Agitated Patients Without Formal Training

Your HEMS teams encounter patients with acute behavioural disturbance, altered mental states, and substance-induced aggression in uncontrolled environments, yet most have never received structured training in managing these situations safely or lawfully.

Legal Exposure Is Growing

Every physical intervention your crew makes must be justified under the Mental Capacity Act 2005, common law necessity, and Human Rights Act obligations. Without formal training, your clinicians, and your charity, carry significant legal liability.

External Training Is Expensive and Unsustainable

Bringing in external trainers for every cohort, every year, across rotating crews is a significant recurring cost. It also means your training schedule depends on someone else's availability, not your operational needs.

Right Care, Right Person Has Changed the Landscape

Since RCRP implementation, police may not always be present when your crews encounter behavioural crisis. Your teams need the skills and legal confidence to manage these situations independently until support arrives.

The Solution

A 4-Day Programme That Builds Permanent Internal Training Capability

Days 1-2 deliver the core clinical skills. Days 3-4 equip your selected clinicians to teach those skills to every crew member, year after year.

Day 1

Legal Framework & Prevention

  • Legal framework for physical intervention in pre-hospital care
  • Mental Capacity Act 2005: Sections 5 & 6 applied to HEMS
  • Dynamic risk assessment for agitated patients
  • The Lawful Excuse framework: six components your crews must evidence
  • Verbal de-escalation: PACE approach and Project BETA model
  • STAMP early warning recognition (Staring, Tone, Anxiety, Mumbling, Pacing)
  • Safe approach and body mechanics in uncontrolled environments
Day 2

Clinical Skills & Confined Environments

  • Restricting movement for rapid tranquillisation and RSI support
  • Clinical holding in confined aircraft and ambulance environments
  • Two-person team techniques tailored to your airframe
  • Head support and C-spine management during behavioural crisis
  • Emergency response to acute behavioural disturbance (ABD)
  • Police liaison: Right Care, Right Person framework and operational scripts
  • Chemical restraint coordination: role clarity between HEMS and police
Day 3

Trainer Development & Facilitation

  • Adult learning principles and facilitation techniques for clinical audiences
  • How to structure and deliver each module with confidence
  • Managing group dynamics in high-stakes training environments
  • Adapting scenarios to your specific aircraft configuration and SOPs
  • Using the trainer manual, slide deck, and assessment tools effectively
  • Building a training calendar that maintains crew currency year-round
Day 4

Assessed Practice & Certification

  • Supervised practice: delivering a full module to peers with live feedback
  • Competency assessment and sign-off process
  • Quality assurance: upholding BILD ACT (RRN) Training Standards when delivering internally
  • Post-course implementation planning for your service
  • Documentation, record-keeping, and governance requirements
  • Ongoing support pathways and annual trainer revalidation
Days 1–2: Core Clinical Skills

The Same Programme Trusted by HEMS Services Across the UK

The first two days deliver our proven Direct Delivery curriculum: the only BILD ACT certified behavioural crisis management programme built from the ground up for the pre-hospital environment. Your trainers learn the content by experiencing it first as participants, ensuring deep understanding before they teach it.

A Complete Trainer Toolkit: Everything Needed to Deliver Internally

Your trainers walk away on Day 4 with everything they need to run the programme independently. No ongoing licence fees. No hidden costs.

Comprehensive Trainer Manual

Step-by-step delivery guide covering every module, with facilitator notes, timing guidance, and key teaching points.

Complete Slide Deck

Professionally designed presentation slides for every session, ready to deliver. Customisable to your service branding.

Ongoing Support

Post-course access to our clinical team for implementation questions, scenario development, and annual trainer revalidation.

The Business Case

Why Train the Trainer Is the Smart Investment

Recoup Your Investment Within 2-3 Cycles

Most services recover the Train the Trainer cost after just 2-3 internal training cohorts. Every subsequent cohort is delivered at a fraction of external delivery fees.

Train on Your Schedule, Not Ours

No more coordinating with external providers. Run training when it suits your crew rotations, operational tempo, and budget cycle.

Maintain Currency Across Every Crew

With internal trainers, you can ensure every clinician, including new starters and rotating staff, stays current without waiting for the next external course date.

Demonstrate Governance & Compliance

BILD ACT (RRN) certification is the gold standard. Having your own BILD ACT Certified Trainers on staff, operating under our affiliate umbrella, gives your board, commissioners, and regulators confidence that your service meets the highest training standards.

Led by Someone Who Understands Your World

Gary Collins

RGN

Gary brings over 32 years of clinical nursing experience and 8 years of specialist trauma management training for armed police officers and tactical medics. He designed this programme from the ground up for the unique challenges HEMS teams face, not as an adaptation of a ward-based course, but as a purpose-built solution for pre-hospital emergency care.

As a BILD ACT Certified Trainer and the author of the forthcoming book “Reading the Room: Understanding Behaviour and De-escalation in Pre-Hospital Emergency Care”, Gary combines deep clinical expertise with an evidence-based approach to behavioural crisis management that is grounded in law, informed by operational reality, and designed to keep both patients and crews safe.

Frequently Asked Questions

Who should attend the Train the Trainer course?+
The course is designed for experienced HEMS clinicians, typically senior paramedics, flight doctors, or clinical educators, who will become your internal training leads. We recommend sending 2-4 people to build resilience into your training capacity.
How is this different from the 2-day Direct Delivery course?+
The 2-day Direct Delivery course trains your crews in the skills themselves. The 4-day Train the Trainer course trains selected staff to deliver that same programme internally, with full materials, assessment tools, and the status needed for your trainers to become BILD ACT Certified Trainers under our affiliate umbrella, delivering the certified curriculum to your crews. It's the difference between buying fish and learning to fish.
What certification do trainers receive?+
Successful graduates become BILD ACT Certified Trainers once your organisation joins the BILD ACT Affiliate network under our umbrella, which we set up as part of this programme. Three things have to align for a BILD ACT certificate to carry the BILD logo: a certified training provider (us), a certified curriculum (this programme), and a certified trainer delivering it (your graduates, under our affiliate umbrella). Once all three are in place, the clinicians your trainers go on to teach receive BILD ACT (RRN) certification, the same nationally recognised standard used across NHS trusts and CQC-regulated services.
Can the course be tailored to our specific aircraft and SOPs?+
Absolutely. During Days 1-2, practical scenarios are adapted to your specific airframe configuration, team structure, and standard operating procedures. On Days 3-4, we show your trainers how to maintain that tailoring for future cohorts.
Where does the training take place?+
We deliver at your base or a venue of your choosing. This allows us to work with your actual equipment and environment, making the training immediately applicable to real operations.
What ongoing support is provided after the course?+
Your trainers receive ongoing access to our clinical team for implementation questions, scenario development guidance, and annual revalidation. We don't disappear after Day 4. We're invested in your long-term success.
How quickly can we start delivering training internally?+
Your trainers can begin delivering to colleagues immediately after successful completion and sign-off on Day 4. Most services run their first internal cohort within 4-6 weeks of the Train the Trainer course.
What is the ROI compared to ongoing external delivery?+
Most air ambulance charities recoup the Train the Trainer investment within 2-3 internal training cycles. After that, every cohort you train internally represents a direct saving versus external delivery fees, travel costs, and scheduling constraints.
